I HIGHLY recommend LasikPlus! I wish I would have done it sooner. I had my free consultation on Thursday and had the procedure done the following day. It’s 3 days later now and it’s so wonderful not having to worry about glasses or contacts. The laser procedure was way quicker and easier than I expected. Dr. Smith and the entire team is amazing! They have your laser eye surgery down to a science! They make it quick, easy, and best of all perfect! I knew how many patients they see on the daily, but also knew Dr. Smith’s reputation. Even though they wasted no time diagnosing my issue and fixing my eye sight perfectly, I felt as if I was cared for at every step in the process. Both Dr. Smith and his team had no issues “slowing” the process to answer each and ever question and/or concern. AND! the results speak for themselves! It was the closest thing I’ve ever experienced to a miracle! I’ve worn glasses and contacts since I was 12 for distance, and at 44 they cautioned me strongly that I would need reading glasses afterward. NOT SO! Dr. Smith has perfected his skill to the point that I see 20/20 both near and far, and they say my vision should get better as I heal fully in the next six months. THANK YOU! Just had Lasik done Friday! The best decision I ever made. I walked out after the procedure seeing great. The staff was amazing & Dr.Smith come in to explain the procedure before we began. I was able to resume my normal life the day after. Still amazed I no longer require glasses and contacts to see 😝

I’m probably the most anxious person ever when it came to Lasik, but after 18 years of glasses and contacts, I was totally over it. I chose LasikPlus because the reviews were amazing and I really wanted the best of the best. I live about six hours away and they got me in for my consult and procedure the same day. Everyone on the team is amazing. Seriously, from my exam, to my procedure and follow up, these are people you *want* to spend time with. I hereby acknowledge and agree as follows:
- My participation in a LasikPlus TeleHealth Consultation (“Consultation”) is completely voluntary.
- I am interested in learning more about laser vision correction (“LASIK”) at LasikPlus. The Consultation will provide me with the opportunity to learn more about LASIK at LasikPlus.
- I understand that I will be speaking to an optometrist and/or non-medical staff during the Consultation, neither of whom are an ophthalmologist.
- The Consultation is not an eye exam. There will be no eye testing conducted during the Consultation. My medical history will not be discussed during the Consultation. I will not be provided with medical advice of any kind during the Consultation.
- If I have medical questions, those questions will not be answered during the Consultation. I can ask medical questions to a doctor when I attend an appointment at a LasikPlus vision center.
- A determination as to my candidacy for LASIK will not be made during the Consultation. Candidacy for LASIK will only be assessed when I attend an appointment at a LasikPlus vision center, undergo the necessary testing, have my eyes examined, and have my candidacy determined by a medical doctor.
- I am at least 18 years of age. I have read, understand and agree with the information contained herein.
