The LasikPlus Vision Center of South Florida (located in Memorial Hospital Miramar) is conveniently located at the junction of Ft. Lauderdale, Miami, and Miramar, FL. Parking is available 24 hours a day, 7 days a week. Free valet parking is available at the main entrance to Memorial Hospital of Miramar – Medical Office Building. Please park only in designated spots or use the valet service.

LASIKPLUS TELEHEALTH CONSULTATION ACKNOWLEDGMENT
I hereby acknowledge and agree as follows:
- My participation in a LasikPlus TeleHealth Consultation (“Consultation”) is completely voluntary.
- I am interested in learning more about laser vision correction (“LASIK”) at LasikPlus. The Consultation will provide me with the opportunity to learn more about LASIK at LasikPlus.
- I understand that I will be speaking to an optometrist and/or non-medical staff during the Consultation, neither of whom are an ophthalmologist.
- The Consultation is not an eye exam. There will be no eye testing conducted during the Consultation. My medical history will not be discussed during the Consultation. I will not be provided with medical advice of any kind during the Consultation.
- If I have medical questions, those questions will not be answered during the Consultation. I can ask medical questions to a doctor when I attend an appointment at a LasikPlus vision center.
- A determination as to my candidacy for LASIK will not be made during the Consultation. Candidacy for LASIK will only be assessed when I attend an appointment at a LasikPlus vision center, undergo the necessary testing, have my eyes examined, and have my candidacy determined by a medical doctor.
